As a human being, I would not call his mother. She is 82, and probably in no shape to handle this. If the sister is estranged, she is concentrating on the mother, and will not or cannot give him any help.
I agree with the other poster, give him the suicide previention hotline. You should try not to get personally involved in this. Let the psychiatrist do his work.

My only other suggestion is to suggest any volunteer organizations, such as the local pet shelter, salvation army, or church that could give him something to do, maybe one or two mornings a week, that he would not have to interact with people if he prefers to be a loner. Sometimes an animal, or a change in routine can help. But not always.
