Unless your tenant has been deemed incompetent, you cannot "make" her seek outside medical services (including placement in a nursing home). You cannot sign her into a nursing home (unless you now wish to be responsible for the cost of such) nor can you expect her physician to do the same.
You can, if you wish, consider terminating her lease based on the conditions of the rental unit. This MAY (and I say only may as I have worked with many stubborn elderly patients who, while having substantial funds, refuse to spend them for home health services/nursing home services) cause her to rethink her living situation options.

The woman is living in squalid conditions, is barely mobile and asking the landlord and other tenants to "take care of her."
