This is a domestic issue between the T's , stay out of it. Only do what is required of you as a LL.
You cannot just remove one party to a lease, based on the request of another party. The T to be removed must request it and you must agree (bad idea) in writing. All parties to the lease should sign if you agree to let one off the hook for his/her responsibilities.
If you remove one and the other defaults, you have no recourse against the one. You have no obligation to remove the one and should not. They both signed the lease and are legally bound by it, do not remove one of them, just because they are fighting. The more people you have to file against, in case you end up in court, the better.
You cannot evict just one T, you must evict the whole kit & caboodle.

Ok so leave the boyfriend on the lease, got it, thank you.
Then what about the new tenant? Should I create an addendum to the lease for the newest roommate? Or...?

If this were to happen to me I would try to get them out. My situation Long distance rental (real estate person handling it "I thought") "boyfriend" tenant called wanted out of the lease. Silly me I said fine. Got it in writing and he sort of said something about her, but I didn't pick up on it. Anyway, pretty soon I find out "girlfriend" is going to jail for selling/using drugs... I'm out rent for several months. After boyfriend "left" I should have given the "girlfriend" notice and got out of the whole mess between them and what ended up to be more of a mess.
