I asked her for the key and she said she had given it to another roommate and was trying to ahold of her to get it back but was having a hard time. I told her that I would need to change the locks and charge them. They were only given one key in the beginning.

Should I have her sign something saying she no longer has possession of the key but that she is vacating, and have the ex boyfriend sign also?

How was she getting in without a key? You can sign a lease termination I guess, but I'm pretty sure both parties have to sign it since both signed the original lease.

She said she was leaving the door unlocked. It is unlocked right now and there is no one there. They have moved almost all of their stuff out except a dresser.
