you said she has been withholding rent and can withhold up to $125 if she can prove repairs. doesn't that mean she actually spent the $125 on repairs, not just that they're needed?
I'd hire a locksmith, go over and change the locks and give her a new key. it will cost you a lot more than her giving you one but at least you'll have access.
I'd then give her proper notice of what I'm doing and when and since she hasn't been cooperative, not really care what she wants.
I also would put her out since she's being a pain - who in the world doesn't want improvements? I wouldn't give new appliances to someone who was less than cooperative.

Yes, the fact she doesn't want to let the LL in makes me wondering what's going on --- extra tenants, extra pets or what. I believe there's something in the lease I use about giving 24 hour notice, but we can still get in to do repairs. Personally if it were me & this is a month to month lease, I would just give notice to terminate the lease - start over with someone else who may appreciate a new roof, appliances and better windows.
