Check your state law. Bryan may be right that you do not need to provide this information in his state of Iowa, but you are legally required to upon request here in Massachusetts.

Even if not, I would err on the side of supplying all the evidence you have. Let the tenant know that you have your stuff together.

You are correct but it is doubtful that the court fees will be less than half the amount of unplugging the toilet and cleaning up. The tenant can not claim she did not contribute to the need for cleaning. She was legally on the lease for the duration and responsible for whatever happened there.
