Yes, you can modify your old lease with those few changes by using a "Lease Addendum" form downloaded from this site.
Another poster said to make a new lease; that can be done also.
I suggest when you mail the "Lease Addendum" form or new lease, that you include the LPA form that says the tenant is giving you notice to leave in 30 thirty days if she does not sign your new lease.
