Anyone have suggestions for a sound proofing underlayment for large format tile on an above grade concrete slab floor. .??

We've tried this mortar product by lacrete which sets up to sort of a rubberized layer but it's way way expensive,........like ninety bucks a bag.

Anyone used cork roll or sheets ?

There are several products that will achieve what you desire, cork underlayment is not one of them as it requires increasing the floor height by the requirement of a layer of cement board and multi layers of thin-set mortar.

You need to be sure exactly what degree of sound proofing you need to achieve first if this is a condo association issue.

The following products when installed correctly will meet the following;

Impact Insulation Class IIC 72 and Sound Transmission Class STC 71

Proflex 90
Super Soundproofing™

Both will run just over $2.00 per foot for materials, but should meet your situation. You will not find the correct products at the big box stores, only from those who specialize in commercial sound proofing.
