I personally use the LPA Application, some minor editing over the years to meet my personal needs or to reflect new information learned here on the forum. The point is the LPA application pretty much states any incorrect, false or missing information is a form of perjury and automatically disqualifies the applicant.
On my app that is one of the statements the applicant(s) have to initial.
