You should have a set of renting guidelines already prepared that includes this sort of thing. You can show them to prospective tenants before they rent from you. This is one way to protect yourself from acusations of being unfair.

If you have a lease, it is also a good idea to have charges for extra people noted so that if it comes up during the lease, you already have a signature that they understand. Of course, this assumes that you are allowed to charge extra in your area.

We charge an extra $25.00 for each adult beyond the second and we also limit the household size based on the number of bedrooms. The new tenant will also have to qualify through our applicaiton process. That is in our lease and in our rental guidelines.

If they refuse or try to dodge the situation, we would send them a 5-day to correct the situation (which usually gets there attention)...although evicting for unapproved tenants is not the easiest thing to evict for...
