None that I know of, unless you check on the internet for one. This is not complicated. Call the T up, and ask him what he wants. You then tell him what you want. If you come to an agreement over the phone, put it in writing, and meet with him, for you both to sign it. If no agreement over the phone, then no use to put something in writing.
