T. Ignore my request to apply for Rent Assistance

NJ has a COVID-19 Rent Assistance Program. For those cannot pay the rent, they can apply for it.
My T. has not paid the rent for 18 months now even before the pandemic, and got the evicted. Unfortunately, due to eviction moratorium, the lockout couldn't be executed. He claimed he got COVID-19, and could not pay the rent.
But, when I ask him to apply for the program, he completely ignore my messages.
I just wanted to leave a record to indicate that he refused to apply the program. What should I say to him, and then can leave such record?

The landlord can apply. Find your state agency that handles it and talk to them.
