The type of notice I am looking for goes above and beyond the typical move out notice that reminds the tenant about leaving the place broom clean and no damages beyond normal wear and tear.

You see, the notice should identify the tenant obligations specifically required for an ESA animal.
HUD website specifies the LL obligations but not surprisingly leaves out the tenant's obligations.

The ESA basically abolishes and redefines the word pet. Because pets have always been about the ES of the person that needs a pet, companion, reality check object.

The ESA program disregards and tramples over a LL's private property rights and contract rights. So this arms the tenant with some power. And unfortunately, there is a rude awakening to the tenant about making sure that there is no residue or odor left to the apartment when they vacate the unit. If there is then the exiting tenant will be charged for the extra coat of paint and refinishing of the floors.

Daniel, there are no regular "forms" out there that will fit your situation. You need to make up your own letter to your tenant, telling them what they must do be able to get their deposit back. Say "cat, dog, or animal" in your letter. Do not use the words "ESA, or pet".

You can require them to have carpets professionally cleaned and deodorized, when they leave, but that's it. You can ask for a copy of the bill from the carpet cleaners and if it shows the carpets were cleaned and deodorized by a professional cleaner, you cannot charge the tenant again for a different professional cleaner, even if the smell is still there.

If you have good hardwood or tile floors, urine should not sink in. You can only charge for painting walls, if the tenants themselves did something to them.
