Daniel, there are no regular "forms" out there that will fit your situation. You need to make up your own letter to your tenant, telling them what they must do be able to get their deposit back. Say "cat, dog, or animal" in your letter. Do not use the words "ESA, or pet".

You can require them to have carpets professionally cleaned and deodorized, when they leave, but that's it. You can ask for a copy of the bill from the carpet cleaners and if it shows the carpets were cleaned and deodorized by a professional cleaner, you cannot charge the tenant again for a different professional cleaner, even if the smell is still there.

If you have good hardwood or tile floors, urine should not sink in. You can only charge for painting walls, if the tenants themselves did something to them.
