this is another post with too much missing info. I'd imagine the lease doesn't say you can "have" a dog ... I'd guess it says in some manner that the landlord MAY approve a dog. HUGE difference.
Did you and your doctor follow whatever is required under the various disabilities laws? A "form" probably isn't good enough.
