Beware office supply store forms! OH requires specific language on those forms! If it is not included, those forms (and the time you spend with them) are useless. You'de have to start over from scratch. Get the forms from Monica to be sure you have the corect ones. You may also need the military affidavit form if Lorain uses that one too. Call the clerk of court and ask which forms you need to include for an eviction (we also have to include the tax card from the auditor here.)

Where do I get a Second Class Statement and Affidavit of Plaintiffs's Claim in Forcible Entry and Detainer? Heck of a long name, I called my local court and they said look online. Thanks for any help
